b. On this page, you will see the different types of cards issued by Myvilla. Just select one
c. Now enter your First Name, Last Name, Current Physical Address (City, State, ZIP Code)
d. Also enter your Date of Birth (you must be 18 years or older to enroll), Social Security Number, email,
e. Go to the next section and provide Email Address, Mobile Phone, Home Phone (optional)
f. Read and agree to the Agreement to Receive Electronic Communications
h. Create a four (4) digit pin and then click on the Next button.

Note that there are two cards which you can apply for and they are MyVanilla Prepaid Visa® Card and MyVanilla Prepaid Mastercard®
